1. Organize your case. Start now by keeping a dated and detailed log
of all phone calls you make or receive, whom you spoke to and their
phone number. Also keep brief notes about what was discussed during your conversations. And when sending mail in response to your case,
send it certified and request a return receipt. Be sure to make copies
of all documents before mailing.

2. Contact the Federal Trade Commission (FTC) - the government agency responsible for maintaining a clearinghouse on identity theft cases and crimes. They are also the best source for downloading the Identity Theft Affidavit and other Identity Theft Worksheets needed to resolve your identity theft case.

3. After obtaining your FTC Identity Theft Affidavit, contact the following credit reporting agencies and alert them to the suspected fraud and request a fraud alert or freeze on your credit report. The freeze option varies by state.

4. File a police report with your local police department - even if you don't think the culprit can or will be caught. Having a police report will help you in clearing your credit report. Try to file the report in person as opposed to an online report. To help with the reporting, bring a copy of the FTC Identity Theft Affidavit you downloaded.

5. Contact your pre-paid insurance plan representative to access your insurance coverage for covered expenses and services.

6. If you believe your social security number has been misused or compromised, contact the Social Security Administration via the internet's Fraud Reporting Form.

7. If you believe your driver license is being used by someone else, contact The Department of Motor Vehicles.

8. If you believe your identity has been stolen via the US mail service contact The U.S. Postal Inspection Service.

9. If you have had your checks stolen or bank accounts set up by an identity thief, contact the major check verification companies. (Also, if you know that a particular merchant has received a check stolen from you, contact the verification company that the merchant uses if it's not listed here:)

10. In addition to obtaining the FTC Identity Theft Affidavit, contact these other agencies for additional recovery assistance:

U.S. Secret Service.

Federal Deposit Insurance Corporation (FDIC).

Visit The Internal Revenue Service, or call 1-800-829-0433 to report tax violations involving the use of your stolen information.

Be sure to refer to your FTC Identity Theft Affidavit, Identity Theft Worksheets and police report when contacting any recovery agency.
